The Role

As a Hospice Admissions Nurse (RN), you will play a vital role in evaluating hospice referrals and guiding patients and families through the admissions process with compassion, clarity, and clinical expertise.

You will be responsible for ensuring accurate eligibility determinations in accordance with Medicare hospice regulations and organizational standards while delivering an exceptional patient and family experience.

Key Responsibilities
  • Conduct comprehensive clinical assessments to determine hospice eligibility
  • Review referral documentation, medical records, physician notes, and diagnostic results to support eligibility decisions
  • Collaborate with attending physicians, hospice medical directors, and interdisciplinary team members to confirm prognosis and appropriateness for hospice services
  • Clearly and accurately document eligibility determinations, including supporting clinical rationale, within the EMR
  • Communicate effectively and compassionately with patients and families during emotionally sensitive situations
What You Bring

Experience:
  • Minimum of 2 years of hospice, palliative care, home health, or acute care experience (hospice admissions experience strongly preferred)
Education:
  • Active, unrestricted California RN license
Skills:
  • Strong understanding of hospice eligibility criteria and regulatory requirements
  • Ability to manage complex and sensitive conversations with patients and families
Soft Skills:
  • Exceptional communication, interpersonal, and clinical judgment skills
  • Professionalism, empathy, and attention to detail
Certifications & Requirements:
  • Current BLS certification
  • Valid driver's license and reliable transportation
