Position Overview

Provides Occupational Therapy services to patients under the direction and supervision of a licensed Occupational Therapist, in accordance with the plan of care and in line with the philosophy, objectives, and policies of the agency.

Responsibilities
  • Implements the treatment plan established by the supervising Occupational Therapist.
  • Provides occupational therapy treatments and interventions as outlined in the plan of care.
  • Assists patients with activities of daily living (ADLs), fine motor skills, and functional mobility training.
  • Instructs patients in the use of adaptive equipment and assistive devices as directed.
  • Documents patient progress, treatment provided, and any changes in patient condition.
  • Reports observations and patient responses to the supervising Occupational Therapist.
  • Communicates effectively with patients, caregivers, families, and agency staff.
  • Instructs patients and/or family members in home programs and safe techniques as directed by the OT.
  • Coordinates care with agency DON and other staff as appropriate.
  • Observes, records, and reports any changes in patient condition to the supervising OT and agency.
  • Participates in supervisory visits with the Occupational Therapist as required.
  • Assures that all tasks are performed in a timely manner.
  • Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
Qualifications
  • Must be a Certified Occupational Therapy Assistant (COTA) licensed by the State of Illinois.
  • Must hold current NBCOT certification.
  • Must have at least one year of Home Health Care clinical experience.
  • Car with required proof of insurance coverage.
  • Maintains annual TB test, CPR certification, and other requirements.
  • Able to read and interpret instructions related to patient care.
  • Able to effectively perform and prioritize multiple functions or tasks as needed.
  • Must be able to travel to patient's place of residence.

Pay

$65,000 - $75,000 per year

Schedule

Monday to Friday;
Weekends as needed

Expected Hours

40 to 50 (Less for part‑time)

Experience

Minimum 1 year Home Health experience required

License/Certification

COTA licensed in State of Illinois with current NBCOT certification
