Position: Home Health Physical Therapist - $80K-120K per year

Overview

Graham Healthcare Group is seeking a Home Health Physical Therapist for a job in Lombard, Illinois. At Residential Home Health and Hospice ("Residential"), we’re looking to add to our extraordinary care team. Our home health physical therapists collaborate to provide management and delivery of patient care plans within a team-based environment.

This full-time position supports patients in Downers Grove and surrounding areas and is eligible for a $20,000 total bonus potential, including a $10,000 first year success bonus and an additional $10,000 sign-on bonus paid within the first 18 months of employment. To receive these bonuses, you must be in good standing, meet performance expectations, and be in the role and status for which you were hired.

Responsibilities
  • Collaborate with the care team to manage and deliver patient care plans in a home health setting.
  • Support patients in Downers Grove and surrounding areas as part of a full-time field role.
  • Participate in an onboarding program and utilize clinical educators, preceptors, and supervisors to guide practice.

Qualifications
  • Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college.
  • Licensed Physical Therapist issued by the state of practice.
  • Minimum of one year healthcare experience.
  • Current driver’s license and ability to spend ~20% of the day driving to/from patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Ability to self-manage time and scheduling.
  • Demonstrated experience with tablets, mobile devices, and EMR software.

Additional Information

Includes compensation related to patient care visits, non-visit activities, paid days off, shift premium pay, and first year success bonus.
