• 12 months of role experience is required with some in the last 12 months.
• Must have at least 3 months of Home Health experience within the past 3 years.
• Experience with Homecare Homebase (HCHB) is preferred from any number of months.

• Candidates must have a Illinois license (required for submission).
• This role will require floating to multiple units
• Local & travel allowed.
• COVID vaccination required after submission. Religious and medical declinations accepted.
• No current placement allowed at Program: AMN Program - Accent Care.

Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.

Float experience does not count towards the unit experience requirement.

Must have homecare homebase documentation, no exceptions.

On Call:
Yes. Monday-Friday.

May be asked to help in other territories within facility’s service area as needed which is about a 60 mile radius from the Facility address.

5 days maximum time off. Facility holidays: 10/31/2026, 11/26/2026, 12/24/2026, 12/25/2026, 12/31/2026. Number of holidays allowed off: 1
