We are looking for candidates with a minimum of 1 year of nursing experience. Those with prior home health experience will be given preference.

• Serve as the patient’s advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care
• Responsible for timely completion and synchronization of visits within the patient home via device (EMR)
• Contributes to the assessment of the health status of individuals or groups.
• Participates in the development and modification of the strategy of care
• Implements the appropriate aspects of the strategy of care and participates in the evaluation of responses to the strategy of care.

• Graduate of an accredited school of professional nursing
• Licensed as an LPN in the state of IL
• One year related experience, including recent Medicare home health experience preferred.
• Must have a reliable car w/current insurance coverage and a current driver’s license
• Experience with Well Sky (Kinnser) EMR system preferred but not required.
