About UsSafe Life Home Healthcare, LLC, a 4-star patient satisfaction organization that has been proudly serving the community since 2010, is seeking a dedicated and experienced RN Clinical Supervisor to join our team in Lombard, IL. This on-site position is essential in overseeing the quality of care provided by our field staff, ensuring coordination with physicians and clinicians to deliver the best possible outcomes for our patients.
Responsibilities- Care Oversight: Monitor and oversee the care provided by field staff to ensure compliance with care plans and safety protocols.
- Coordination: Collaborate closely with physicians, clinicians, and other healthcare professionals to develop and implement patient care plans.
- Patient Advocacy: Serve as a liaison between patients, families, and healthcare providers to advocate for the patient’s needs and preferences.
- Documentation: Maintain accurate and timely records of patient care, including assessments, care plans, and follow-up documentation.
- Compliance: Ensure all care provided meets state and federal regulations as well as company policies.
- Experience: Minimum of 2 years of experience as a Home Health RN.
- EMR Proficiency: Experience using Well Sky (Kinnser) EMR for home health is preferred.
- Licensure: Current RN license in the state of Illinois.
- Skills: Strong organizational, communication, and problem-solving skills. Ability to work independently while managing multiple priorities.
- Team Player: Ability to work collaboratively with a multidisciplinary team.
