Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Graham Healthcare Group is seeking a Registered Nurse (RN) for home‑health services in Lombard, Illinois.

Job Overview

This ongoing, full‑time position supports patients in Downers Grove, IL and surrounding areas. Eligible for a $10,000 first‑year success bonus.

Responsibilities

As a home‑health registered nurse case manager, you will lead and collaborate with a team-based environment to manage and deliver patient care plans.

Qualifications
  • RN license in the state where you will work
  • Graduate of an approved school of practical nursing
  • At least one year of RN experience and clinical competence to deliver quality patient care
  • Current driver’s license and ability to spend approximately 20% of your day driving to and from patient locations
  • Commitment to consistently meet critical deadlines for charting
  • Self‑management of time and schedule
  • Experience with tablets, mobile phones, and EMR software
Drug Screening

Successful completion of a drug screen, including medical and recreational marijuana limits, is required prior to employment.
