Bella Terra Lombard is seeking a MDS Coordinator to join our skilled nursing and rehabilitation facility in Illinois. You will develop, monitor, audit, and modify each resident's care plan, performing resident assessments and aiding in discharge planning.
Applicants should have an Illinois RN license, CPR certification, and be a graduate of an accredited nursing program. Prior MDS Coordinator experience is preferred; one year RN and one year CNA experience at Bella Terra Lombard is a plus.
