LPN/LVN – Home Health

Under the supervision of a Registered Nurse, the LPN/LVN (Licensed Practical/Vocational Nurse) – Home Health provides skilled one-on-one nursing care to patients in their home and accompanies them when indicated to appointments, etc.

• Documents each shift or visit to reflect accurately the provision of skilled nursing care rendered in accordance with physician's orders.
• Changes in patient condition are identified and documented with resolution on the appropriate clinical form. Director/Supervisor notification if applicable
• Physician notification to obtain necessary orders for intervention(s) per state regulations
• Performs specific treatments and medication administration in accordance with physician orders and home health agency's protocols
• Documentation reflects patient collaboration and participation in their plan of care
• Requests for modification of orders and additional orders are made based on the patient's clinical status and skilled nursing judgment
• All physician collaboration/coordination of care is documented in the medical record and communicated as needed
• Provides patient and family education as appropriate
• Attends case conferences, team meetings and education in-services as required

• Graduate of an accredited practical or vocational nursing program
• Currently licensed as a Licensed Practical/Vocational Nurse in the state in which the LPN/LVN will practice
• One year prior professional nursing experience, home care experience preferred
• Complies with all relevant professional standards of practice
• Participation and completion of Amergis' Competency program when applicable
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Successful completion of new hire training as applicable to job site
• Understand patient confidentiality and HIPAA requirements
• Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
• Computer proficiency required
• Must be at least 18 years of age
