In this vital role, youll utilize your expertise in home health nursing to provide assessment, planning, and care management for individuals requiring medical assistance outside a hospital setting. • Current and unrestricted RN license
• Previous experience in Home Health or similar community-based nursing care
• Strong assessment and critical thinking abilities
• Proficiency in developing and implementing individualized care plans
• Exceptional communication and documentation skills
• Reliable transportation and valid drivers license

• Conduct in-home assessments to evaluate patient needs, safety, and care preferences
• Develop and maintain comprehensive care plans tailored to each patients condition and goals
• Administer medication, wound care, and prescribed treatments as directed by healthcare providers
• Educate patients and families on managing health conditions, medications, and prevention strategies
• Monitor and document progress, updating care plans as needed in response to changes
• Coordinate with interdisciplinary teams to ensure continuity and quality of care
• Maintain accurate records in compliance with healthcare regulations and standards
