Licensed Practical​/Vocational Nurse

As a Licensed Practical/Vocational Nurse at Sevita, you'll deliver person-centered nursing care in home and community-based settings for individuals of all ages and abilities. Responsibilities include assessing health status, administering medications and treatments, monitoring vital signs, documenting in EHR systems, and coordinating care with an interdisciplinary team. You'll educate individuals and families, support rehabilitation and daily living needs, and uphold strict infection control and safety standards.

Responsibilities

• Provide direct nursing care including medication administration, treatments, and monitoring of vital signs
• Perform ongoing patient assessments and report changes in condition to the supervising RN or provider
• Document care, observations, and interventions accurately in electronic health record systems
• Support individuals with daily living, rehabilitation, and behavioral health needs in home and community-based settings
• Educate individuals and families on care plans, medications, and health maintenance
• Collaborate with an interdisciplinary team to develop and implement person-centered care plans
• Maintain infection control, safety, and regulatory compliance at all times
• Assist with coordination of appointments, community services, and follow-up care
• Communicate empathetically and professionally with individuals, families, and team members
• Participate in ongoing training and professional development offered by Sevita
