National Youth Advocate Program (NYAP) is hiring a Care Coordinator for the Ohio Rise program in Shelby County, OH. The role delivers community-based wraparound coordination for children with complex needs, guiding families through the Ohio Rise process and coordinating with providers and community resources to improve outcomes.
Under supervision, you will support families to stay safely at home and in their communities, while reducing out-of-home placements.
