Medical Assistant - Personalized Care

Role Overview:

The Person alized Care Medical Assistant serves as a primary contact for program members, delivering proactive, relationship-centered healthcare to enhance patient satisfaction, engagement, and outcomes. They assist physicians during exams, perform intake, document vital signs, update electronic medical records, and support preventive and chronic care management. Additionally, they respond to patient communications, conduct pre-visit planning, coordinate follow-ups, and maintain a safe, organized clinical environment.

The role involves proactive outreach post-hospital visits and supporting front office tasks as needed.

Candidates should have a high school diploma or GED, healthcare program graduation (e.g., Medical Assistant, CNA, LPN, EMT), and current certifications including BLS. Prior Medical Assistant experience, preferably in family or internal medicine, is required. Strong computer skills, EMR experience, and familiarity with practice management systems are preferred. The role demands excellent communication, empathy, professionalism, and the ability to work effectively in a fast-paced environment.

Salary ranges from $21 to $27 per hour, based on experience and skills.
