Family Practice Nurse Practitioner

We are seeking a dedicated, forward-thinking Family Practice Nurse Practitioner to join our team in Lone Tree, Colorado, and help our patients live their biggest, healthiest lives.

We are looking for a full-time, experienced Family Practice Nurse Practitioner (APRN) who is open to a more integrative and wholistic health care model. In this role, you will manage a dedicated patient panel spanning pediatric, adolescent, and adult populations aged 5 to 64.

Because we operate outside the constraints of traditional fee-for-service models, you will have the time to truly listen, and build lasting partnerships with families and individuals. You will handle everything from routine pediatric care and preventive wellness to complex adult chronic condition management, lab analysis and acute same‑day visits.

Comprehensive Primary Care:
Manage an independent, full-spectrum patient panel (ages 5–64), performing all standard primary care functions with an integrative approach.

Chronic Disease & Medication Management:
Safely manage, de‑prescribe, or optimize chronic disease protocols and medications for pediatric, adolescent, and adult patients.

Lab Interpretation:
Analyze and interpret clinical labs

Acute & Same-Day Care:
Diagnose and treat acute illnesses, infections, injuries, and urgent medical needs across all age groups in your panel.
