JOB SUMMARY

The Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by State Medication Administration Regulations.

POSITION OVERVIEW

  • Administer and document regularly prescribed medications per the Medication Administration record, observing the five rights for safe administration and physician orders.
  • Order medications from the pharmacy when and as prescribed, 7 days prior to running out, and as requested by the Wellness Director to ensure residents always have medications available. Notify the Wellness Director within 24 hours if not received.
  • Immediately after administering medication, document it in the resident’s medical record. Date and initial the medication pack after use.
  • Observe and report to the licensed nurse any reactions and side effects of medication administered to residents.
  • Take and record vital signs prior to administration of medications that could affect or change vital signs as ordered by the Physician or Wellness Director.
  • Report any medication error to the Wellness Director immediately.
  • Assist with development and review of the resident’s care plan in conjunction with other disciplines.
  • Ensure residents privacy, respect, and dignity.
  • Detect, correct, and report unsafe conditions that may result in harm to a resident.
  • Assure physical comfort, safety, and mental well‑being of residents.
  • Assist in daily care tasks such as toileting, grooming, bathing, feeding, dressing, observing food intake, hair and nail care, moving residents, and response to calls.

  • License/Certification: Completed state‑approved Medication Administration training, First Aid certification as required by ALF regulations, and QMAP certification in CO.
  • Education/

    Experience:

    Minimum high‑school diploma or equivalent. Preferred experience in assisted living or memory care with a strong work record and medication administration experience.
  • Continuing

    Education:

    Required by law and monthly in‑service education and training within the community.
  • Other: Must read and write English, interpret safety rules and operational manuals, demonstrate good communication skills, show sound judgment, work with licensed nurses within delegated tasks, and not exceed limitations.
  • Job Knowledge: Perform medication administration functions, understand procedures, and apply universal precautions.
  • Industry

    Experience:

    Minimum three years’ experience in senior living and a passion for working with seniors.
  • Team members scheduled 24+ hours per week are eligible for benefits; families of such team members are also eligible.
