Anchor Co-Living is seeking a compassionate Care Coordinator to support Recuperative Care, Housing Navigation, and Enhanced Care Management. You will guide members through recovery, connect resources, and help them secure stable housing while documenting all steps in the EMR.
You'll coordinate transportation, housing applications, and referrals, collaborate with healthcare providers and community partners, and ensure timely updates to care plans and MCP reporting.
