Job Summary and Responsibilities
As our Palliative Care Nurse Practitioner you will support patient and physician satisfaction by performing as a Physician Liaison and Patient Educator so our patients can achieve optimal health outcomes while benefiting from ethical, professional, and compassionate care.

The Clinical Coordinator supports patient and physician satisfaction by performing as a Physician Liaison and Patient Educator.
- Physician Liaison - Identifies need for Case Management services through gathering and evaluation of relevant data.
- Coordinates patient's care from admission, through hospitalization, to post-discharge follow-up, in collaboration with the healthcare team to best meet the patient's physical, emotional, evaluation and revision of the plan to achieve optimum patient outcomes.
- Patient Educator:
Support Core Measure Education Requirements. - Support any Regulatory Agencys Required Patient Education.
- Support Patient Satisfaction Initiatives and Teaching Requirements including New Medications and Potential Medication-Related side effects, pain control, illness related symptoms post discharge and how to obtain help if needed.
- Evaluate any knowledge deficit and education patient as needed using teach-back principles.
- Collaborate with physicians, nursing staff, Administration, Case Management, Quality Improvement, Nursing Education and Patient Satisfaction Champions.
- Two to five years acute hospital experience as a practitioner of nursing and proven competence in adult patient clinical areas.
- Hospice, Palliative Care or Skilled Nursing experience
- Experience with teaching and/or mentoring an asset.
- Experience in initiation and/or application / utilization of research.
- Current California Nurse Practitioner license
- BLS required.
- MSN
- Advanced preparation in pain management.
- Experience with teaching and/or mentoring an asset.
