Position Summary

The Charge Nurse - LVN/LPN supervises the day-to‑day nursing activities of a facility or unit in accordance with federal and state regulation, company policies and procedures, and nursing standards to provide the highest quality of nursing care to our residents.

Duties and Responsibilities
  • Directs and supervises day‑to‑day functions of assigned personnel and ensures that policies and procedures are followed. Conducts staff performance appraisals and is responsible for coaching, counseling, and disciplining staff. Makes recommendations to supervisor concerning wage and salary adjustments, hiring, promotion, termination, transfers, etc.
  • Oversees implementation of resident plan of care and evaluates resident response.
  • Completes routine rounds during shift to observe residents, to determine if nursing needs are being met and to ensure nursing personnel are performing work assignments in accordance with acceptable nursing standards, facility policies and procedures, and governmental regulations.
  • Initiates emergency support measures as needed.
  • Monitors treatment schedules to ensure they are provided as scheduled and may perform duties of Treatment Nurse.
  • Completes requisitions and arranges for diagnostic and therapeutic services, as ordered by physician, and in accordance with facility's established procedures. Schedules tests and completes preps as indicated.
  • Contributes to formulation/review of nursing care plans weekly and PRN.
  • Provides resident/caregiver instruction and education as necessary.
  • Encourages independence by resident in activities of daily living to fullest extent possible.
  • Receives and gives change of shift report upon reporting in and ending shift. Ensures information provides continuity of care.
  • Performs documentation responsibilities in accordance with company requirements. Completes accurate, thorough and timely admission records, routine resident observations/transfer notes, death/discharge summaries and changes in resident's physical/psychological condition in accordance with facility policies and procedures.
  • Monitors record‑keeping of staff members for accuracy, thoroughness, completeness and timeliness.
  • Prepares and administers medications as ordered by physician in accordance with governmental regulations and facility policies and procedures.
  • Orders medications upon admission, new medication orders and per pharmacy re‑fill procedures. Follows facility procedure for administration and counting of narcotics. Ensures that policies and procedures for administration, storage and control of medications and supplies are being followed rigorously. Ensures medications are charted fully, accurately and in a timely manner.
  • Coordinates with resident's physician in the care of resident including notification upon change of condition.
  • Makes rounds with physicians as necessary.
  • Identifies resident safety concerns and takes appropriate actions to maintain a safe environment including but not limited to: resident identification bands and allergy bands (if applicable), side rails and height of bed are adjusted; resident call light and equipment is within reach; restraints when used are maintained properly; rooms are neat and orderly; all emergency equipment can be readily located and operated (emergency oxygen supply, drug box, fire extinguisher, etc.)

    Knowledge, Skills, Abilities & Qualifications
  • Nursing degree from accredited college or graduation from approved LVN program.
  • LPN/LVN license required.
  • Valid Adult Basic First Aid and CPR Certification preferred.
  • A minimum of 1–3 years of experience in direct patient care, preferably in acute or long‑term care setting, with responsibility for providing nursing care to multiple residents preferred; previous Charge Nurse or supervisory experience preferred.
