Oncology clinic is attached to the chemo treatment center and the hospital, which will require providers to provide two years of inpatient case logs to qualify for privileges. Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation.

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties. ________________________________________________________________ ADDITIONAL DETAILS If the provider has a clean file, we can pursue temporary privileges for a December start if not, we may be looking for more January.

Providers with settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges.
