General Position Summary

The CalAIM Housing Navigator is responsible for providing Housing Navigation, Housing Deposit, and Housing Tenancy and Sustaining Services to eligible Medi-Cal participants experiencing homelessness, housing instability, or at risk of losing housing. The Housing Navigator works collaboratively with clients, landlords, property managers, community partners, healthcare providers, and social service agencies to identify and secure safe, stable housing while supporting participants in maintaining long term housing stability.

This position requires strong case management, advocacy, resource coordination, and documentation skills to ensure compliance with CalAIM Enhanced Care Management (ECM) and Community Supports requirements, contractual obligations, and organizational policies.

Job Responsibilities
  • Conduct housing related assessments and develop individualized housing plans that identify participant needs, barriers, strengths, and goals while coordinating services that support successful housing placement and stability.
  • Assist participants in locating, applying for, and securing safe and affordable housing by providing housing navigation, advocacy, landlord engagement, application support, and linkage to community housing resources.
  • Coordinate and facilitate Housing Deposit services, including eligibility determination, documentation, and payment of approved housing related expenses such as security deposits, utility deposits, application fees, and essential move in needs.
  • Provide Housing Tenancy and Sustaining Services by educating participants on tenant rights and responsibilities, promoting independent living skills, supporting access to community resources, and implementing interventions that prevent housing loss and promote long term stability.
  • Collaborate with healthcare providers, managed care plans, community partners, landlords, and other service agencies to ensure comprehensive care coordination and successful participant outcomes.
  • Maintain accurate and timely documentation of assessments, service plans, participant contacts, referrals, housing outcomes, and other required activities in accordance with CalAIM, Medi-Cal, and organizational requirements.
  • Monitor participant progress, evaluate housing stability outcomes, conduct follow up visits and outreach activities, and adjust service plans as necessary to meet participant needs and program objectives.
  • Participate in team meetings, trainings, quality improvement activities, and community engagement efforts while ensuring compliance with all applicable federal, state, county, health plan, and organizational policies and procedures.
Minimum Requirements
  • High School Diploma or equivalent.
  • Experience working in case management or a related field, preferably with a background in healthcare or social services.
Other Requirements
  • Valid driver’s license and proof of current auto insurance.
  • Reliable vehicle with appropriate registration and inspection for travel to client locations and other work-related duties.
  • Bilingual, bi-literate, English/Spanish, English/Punjabi, English/Hmong, and/or other language ability based on program needs preferred.
  • State regulations require a physical examination, current tuberculosis test, and verified fingerprint and criminal records clearance before beginning work.
