Job Summary

The Geriatric Social Worker will join a team of professionals to support veteran residents in the Veterans Support to Self-Reliance (VSSR) Pilot program, overseeing intake, assessments, and care planning.

Responsibilities
  • Connect veterans with appropriate senior resources.
  • Provide counseling, support, and intervention related to behavioral management, caregiver stress, and coping strategies.
  • Assist veterans to apply for and manage benefits, and utilize community resources.
  • Coordinate medical appointments and transportation, and accompany veterans as needed.
  • Lead medication management support.
  • Help veterans connect with tele‑health services.
  • Develop and monitor care plans, revising as necessary on a bi‑annual basis.
  • Schedule discharge meetings, discuss long‑term care plans with families and the Program Director.
  • Act as a liaison with caregivers, community services, and the inter‑disciplinary team.
  • Monitor participant attendance and case mix and center daily attendance.
  • Facilitate weekly and/or monthly support groups when appropriate.
  • Coordinate transportation and peer specialist staff for benefit and assistance activities.
  • Provide training to case management and property management staff on aging‑related health declines.
  • Support Pilot staff with resident interactions and trauma‑informed communications.
  • Identify resident needs such as accommodations for disability and income assistance.
  • Report suspected abuse to appropriate monitoring agencies.
  • Provide company vehicle transportation for offsite activities as needed.
  • Complete internal and external reporting for grant funders.
  • Perform other assigned duties.
Qualifications

Education

  • Master’s in Social Work or required credentials for Marriage and Family Therapist or Licensed Professional Clinical Counselor, as issued by the California Department of Consumer Affairs.

Experience

  • One year of social work experience with a public or non‑profit agency serving vulnerable and underserved groups.
  • Experience in counseling, support groups, and participant assessments (preferred).
  • Experience in adult day support or residential services (preferred).

Physical Requirements

  • Ability to safely perform manual movements needed for job functions.
  • Ability to carry out instructions promptly and accurately.
  • Flexibility, positivity, and cooperation in problem‑solving and teamwork under changing conditions.
  • Ability to move around the facility and sit at a workstation; reasonable accommodations available for disabilities.

Additional Qualifications

  • Experience working with diverse populations and trauma survivors.
  • Sensitivity to complex psychosocial and substance abuse issues.
  • Case management, outreach, and home visits experience.
  • Adaptability between clinical case management and evidence‑based, trauma‑focused psychotherapy.
  • Strong communication skills with at‑risk clients and participation in the agency’s DEI initiative.
  • Valid California driver’s license and auto insurance; travel among housing sites required (mileage reimbursed).
  • Driver history report meeting company insurance requirements and completion of a driver training course.
