Your Role: FSP Case Manager
Location: Long Beach
Pay: $22.56/Hourly
Your Toolkit Should Include:
- Bachelor's degree in Psychology, Sociology, Human Services, or a related field; 2 years of social services experience may substitute for the degree
- At least 2 years of direct service experience working with emotionally disturbed children and/or transitional-age youth
- 1+ year of case management experience preferred
- Bilingual in English and Spanish, with the ability to read, write, speak, and understand both languages
- Valid driver's license, current auto insurance, and reliable transportation
- Strong communication, documentation, organization, and problem-solving skills
- Ability to work independently, respond calmly to challenges, and collaborate effectively as part of a team
- Understanding of the emotional, developmental, and cognitive needs of children, youth, and families
- Commitment to confidentiality, ethical practice, mandated reporting requirements, and agency policies
Your Superpowers Include:
- Build meaningful relationships with clients and families while helping them work toward their individual goals
- Provide case management, collateral support, behavioral interventions, developmental education, and resource linkage
- Meet with clients in their homes, schools, and communities, typically 1-2 times per week or as needed
- Partner closely with therapists, families, schools, community providers, and other members of the treatment team
- Help families access resources such as housing, healthcare, social services, extracurricular activities, respite, and other community supports
- Facilitate and participate in Child and Family Team Meetings and other collaborative planning meetings
- Teach positive coping skills, support self-advocacy, and help families develop practical strategies for navigating challenges
- Complete required assessments, progress notes, outcome measures, flex-fund requests, referrals, and other documentation in a timely manner
- Advocate for client and family rights, safety, and access to appropriate services
- Collaborate with the treatment team to monitor progress, identify changing needs, and support successful transitions and discharge planning
- Maintain approximately 80% billable service delivery (26 hours per week)
- Participate in staff meetings, trainings, supervision, and professional development opportunities
- Help connect families with the tools, resources, and support systems they need to thrive
