Social Services Program Team Lead
WORK LOCATION:
SMRC, Long Beach
PAY: $47,890.79
SCHEDULE/
SHIFTS:
Monday – Friday 8:00 a.m. to 4:30 p.m.
Supervisory-level position accountable for administrative work involving professional social services, which encompasses the provision of case management services designed to strengthen, rehabilitate, and preserve individuals and/or families; and provide protection and care for children and vulnerable adults in need of supplementary, substitute care, or provide full-time intake services. Employee supervision includes hire recommendations; training; work planning, assignments, and review; and performance assessment, coaching, and progressive discipline.
Supervision at this level also requires these employees to have the knowledge and skills to perform and review the work overseen.
RESPONSIBILITIES:
- Serves as Support Coordinator (SC) for people enrolled in the Home and Community-Based /DD Waiver program.
- Manages and maintains caseload of people receiving /DD Waiver services.
- Assists Director in overall direction of the /DD Waiver Support Coordination Department
- Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload.
- Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative.
- Coordinates and monitors identified supports and services through regular monthly contact, face to face quarterly reviews, and annual assessment and re-certification meetings.
- Ensures services are adequate, appropriate, and meet the person's needs.
- Travel throughout the six southern counties will be required.

Preferred Qualifications Typically requires a Bachelor's Degree and 4-7 years of experience. Agency preferred qualifications:
Degree and 4-7 years of experience and one year of experience working with people with IDD. Please attach your proof of education. Must be eligible to drive a State of Mississippi vehicle.
