Social Services Program Coordinator III
Work Location:
1170 West Railroad St., Long Beach, MS 39560 / HCBS-Waiver, Support Coordination
Pay: $40,286.40
Schedule/
Shifts:
Monday – Friday 8:00 a.m. to 4:30 p.m.
Responsibilities:
- Serves as Support Coordinator (SC) for people enrolled in the Home and Community-Based /DD Waiver program.
- Manages and maintains caseload of people receiving /DD Waiver services.
- Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload.
- Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative.
- Coordinates and monitors identified supports and services through regular monthly contact, face to face quarterly reviews, and annual assessment and re-certification meetings.
- Ensures services are adequate, appropriate, and meet the person's needs.
- Travel throughout the six southern counties will be required.

Preferred Qualifications:
- Requires a Bachelor's Degree and 4-7 years of experience and one year of experience working with people with IDD.
- Must be eligible to drive a State of Mississippi vehicle.
Special
Qualifications:
- Bachelor's degree or above in an IDD-related field and no related experience.
- Bachelor's degree or above in an unrelated field and at least one year of IDD-related experience.
