Legacy Lifecare VNA/Hospice seeks a Registered Nurse Case Manager to deliver compassionate, resident-centered care in home and assisted living settings. This full-time role involves 40 hours weekly, Monday to Friday 8:00 AM–5:00 PM with occasional on-call rotations; salary ranges $41–$43 per hour, depending on experience.
The RN will observe and report changes, perform basic care, support palliative and hospice needs, and coordinate care with families and facilities.
