Legacy Lifecare VNA/Hospice is seeking aRegistered Nurse Case Manager to provide compassionate care to residents in home or assisted living settings. The RN will monitor responses to medications and treatments, perform basic resident care, and support palliative and hospice needs.
The ideal candidate holds a MA RN license, has at least 1 year of RN experience, and is willing to travel to patient homes or hospice facilities. Fluent English communication is required.
