- Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
- Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
- Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
- Connect residents and families with community resources and post-discharge support services to promote successful care transitions.
- Bachelor or Master of Social Work (BSW or MSW)
- Current, unencumbered MA Social Worker license (LSW)
- Prior experience in a long‑term care setting strongly preferred
- Ability to communicate fluently in English - both verbally and written
