Optimal Home Care is looking for a Registered Nurse to provide services to our patients. This position has a $10,000 sign-on bonus.

Position Details

The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes. When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.

Pay: $80,

Responsibilities and Qualifications
  • Provide home health nursing care to patients in accordance with physician orders and patient needs.
  • Assess patient condition and develop, implement, and evaluate individualized nursing plans of care.
  • Coordinate with interdisciplinary team to ensure quality patient care.
  • Document patient activity and communicate effectively with clients and families.
  • A graduate of an accredited, approved school of professional nursing
  • Active license in the State of Colorado in good standing
  • Must be able to communicate in the language of the client and document client related-activity as appropriate
  • Current driver’s license
  • At least one (1) year of experience as a Registered Nurse. Home Health experience is a plus.
  • Familiarity with OASIS preferred
