• Active Licensed Practical Nursing license in CO
• 2+ years’ geriatric/home‑care experience
• Solid clinical assessment skills and a passion for problem‑solving
• CPR/First Aid certification and a valid driver’s license
• Warm, professional communication that builds trust with families and field staff
