Care Manager

Life Care Centers seeks a Care Manager, Registered Nurse, to coordinate and oversee clinical care for senior residents in a rehabilitation and long term care setting. In this role, you will complete assessments, develop and update care plans, monitor resident progress, and collaborate with an interdisciplinary team to ensure safe, high quality outcomes. You will educate residents and families, support discharge planning, and help lead quality and compliance initiatives.

Responsibilities

• Conduct comprehensive nursing assessments for new and existing residents
• Develop, implement, and update individualized care plans
• Coordinate interdisciplinary care with physicians, therapists, and social services
• Monitor resident conditions and respond to changes in status
• Educate residents and families on care plans, treatments, and discharge needs
• Ensure compliance with clinical standards, policies, and regulatory requirements
• Document care, assessments, and outcomes accurately and timely in records
• Participate in quality improvement and patient safety initiatives
• Support care transitions, including admissions, discharges, and referrals
• Mentor and support clinical staff in best practices for resident care
