Unit Care Coordinator

Life Care Centers is seeking a Unit Care Coordinator RN to lead and coordinate patient care on our skilled nursing unit. In this role, you will oversee clinical care plans, assess resident needs, administer medications, and ensure regulatory compliance. You will collaborate closely with an interdisciplinary team, support CNAs and nurses, and communicate with families regarding care goals and progress. Responsibilities

• Coordinate and oversee daily care for residents on assigned unit
• Develop, implement, and update individualized care plans
• Perform comprehensive nursing assessments and monitor resident status
• Administer medications and treatments per physician orders
• Ensure compliance with state, federal, and facility regulations
• Supervise and support nursing staff and CNAs on the unit
• Document care, assessments, and interventions accurately in the EMRCommunicate with residents and families regarding care and progress
• Collaborate with interdisciplinary team to optimize outcomes
• Participate in quality improvement and patient safety initiatives
