Care Manager; Registered Nurse​/RN

Role Summary: The RN Care Manager coordinates patient care and services to ensure quality outcomes. They collaborate with patients, families, care teams, payers, and external entities to facilitate safe transitions between care settings, adhering to all legal and organizational standards.

• Develop clear, organized written communication and manage time effectively
• Serve as a liaison with external case managers, physicians, families, and community resources
• Train and educate patients, families, and care staff on care plans and procedures
• Implement care management standards, policies, and ethical practices
• Ensure compliance with laws, regulations, and organizational policies
• Maintain confidentiality and uphold professionalism and integrity
• Utilize computer systems for data entry and retrieval
• Assist in patient evacuation and provide excellent customer service
• Participate in interdisciplinary team communication and decision-making
• Perform physical activities such as lifting and moving patients as needed

• Associate’s or Bachelor’s degree in Nursing
• Active RN license in good standing
• Preferred clinical experience in post-acute care, case management, or discharge planning
• Optional certifications: CCM or RN BC in Nursing Case Management
• Strong organizational, decision-making, and communication skills
• Knowledge of regulations governing post-acute care facilities
• Ability to work independently and as part of a team with professionalism and integrity
