Wellness Nurse

Morning Star Senior Management LLC seeks compassionate LPNs/RNs to provide high-quality clinical care for residents in our senior living community. The Wellness Nurse completes assessments, administers medications and treatments, monitors chronic conditions, and responds to urgent needs on NOC shift. Partner with care teams to develop and update care plans, document accurately, and communicate with families and providers. Support a faith-inspired, servant-leadership culture by mentoring caregivers, promoting dignity and safety, and ensuring compliance with state regulations and company policies.

PT and FT roles with growth opportunities.

• Provide direct nursing care, including assessments, treatments, and medication administration for senior living residents on NOC shift
• Monitor resident conditions, respond to changes in status, and coordinate follow-up with providers and families
• Develop, implement, and update individualized care plans in collaboration with interdisciplinary team members
• Maintain accurate and timely documentation in accordance with regulatory standards and company policies
• Supervise, coach, and support caregivers to ensure high-quality, compassionate resident care
• Promote resident safety, dignity, and comfort while upholding infection control and quality standards
• Communicate effectively with residents, families, and team members to support trust and satisfaction
• Participate in rounds, care conferences, and shift handoffs to ensure continuity of care
• Support and model the community’s faith-inspired, servant-leadership culture and values
• Assist with training, onboarding, and ongoing education for care staff as needed

• Active Colorado LPN or RN license in good standing
• Current BLS/CPR certification
• Experience in senior living, long-term care, or assisted living preferred
• Knowledge of state regulations for assisted living and memory care
