Provides and assists with personal care services and related tasks in accordance with the patient's plan of care as assigned by clinical leadership. Tasks may include: assisting patient with ambulation, assist with personal hygiene, administration of nutritional support and/or medications as needed. Reporting of patient condition and needs to the Case Manager or Director of Nursing.
Requirements
Documentation Requirements
- All visit documentation must be completed within 24 hours of visit
- Charting must be thorough, individualized, and reflective of patient condition
- Communication of concerns must occur via Teams and/or direct contact with the RN Case Manager
- All required forms and care plans must be updated timely
- Current CNA certification in the State of Texas
- Minimum one (1) year of healthcare experience preferred (hospice experience a plus)
- Current CPR certification
- Valid Texas driver's license and reliable transportation
- Ability to pass background check and drug screening
- Strong communication and interpersonal skills
- Ability to work independently in home and facility settings
- Ability to lift, transfer, and reposition patients
- Ability to bend, stoop, and stand for extended periods
- Ability to travel between patient locations
