Position Objective

Job SummaryAs an AIDE of Kindful Health you will work with the RN Case Manager to provide best‑in‑class care to a reasonably sized caseload. The AIDE will present at IDT every 2 weeks with updates from the care team and coordinate with the RN Case Manager, Clinical Director and Medical Director to assure best‑in‑class care.
EssentialEducation and Experience
Must be a certified nurse aide with a current license and current continuing education.
Responsibilities & Duties- Adheres to all regulatory requirements and professional standards
- Follows policies and procedures consistent with recognized standards of professional practice
- Identifies system problems or inefficiencies that may negatively affect patient care
- Manages clinical excellence within your patient census.
- Provides high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
- Collaborates with the RN Case Manager, Clinical Director, Medical Director and entire team to provide a “concierge” level hospice experience

CompensationHourly – $19 – $21 per hour.
