Seven Healthcare is pleased to present a rewarding opportunity for a Travel Care Manager RN in Longview, Texas. This assignment offers a weekly blended rate of $2,100, based on three 12‑hour night shifts. This role is perfect for an RN with strong clinical judgement, exceptional coordination skills, and experience supporting patients through complex transitions of care.

• Perform comprehensive assessments to determine discharge needs and barriers
• Develop patient‑centered care plans tailored to clinical, psychosocial, and post‑acute needs
• Coordinate with providers, therapy, pharmacy, and community resources
• Facilitate transitions to SNF, rehab, home health, DME, or outpatient services
• Conduct utilization reviews and work with insurance payors
• Provide education to patients and families regarding follow‑up care
• Maintain clear, timely, and compliant documentation
• Advocate for patient safety, quality, and continuity of care

• Active Texas RN license or compact eligibility
• Current BLS certification
• Minimum 1–2 years’ Care Management or Case Management experience
• Strong interpersonal communication and clinical assessment skills
