- Doctorate Degree from an accredited program
- Current TX Licensure
- BLS (AHA or ARC) Certification
- 1 year inpatient rehab experience
- 2 years’ experience as a Physical Therapist
- Experience with EMR Medhost (preferred)
- Must include permanent home address and SS# on the submission
