Home Health - Full Time Physical Therapist

Frontpoint Health is a fast-growing home health agency committed to delivering personalized, high-quality care. We're currently seeking experienced and compassionate Physical Therapist (PT) to join our team and help patients recover safely and comfortably in their homes.
What You Will Do- Conduct patient assessments and assist the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests.
- Assist in the development of the Plan of Care.
- Direct and aid patients in active and passive exercises, muscle re-education, and gait, functional, ADL, transfer, and prosthetic training.
- Treat patients to relieve pain; develop and restore function; and maintain maximum functioning using exercise, massage, heat, water, light and electricity.
- Use ultraviolet and infrared lamps, diathermy, ultrasound, whirlpool, contract baths, and apply moist packs.
- Observe, record, and report to the physician and supervising therapist the patient’s response to treatment and changes in the patient’s condition.
- Instruct patients, significant others, and staff on the care and use of wheelchairs, prosthetics, and orthopedic devices; and in areas of physical therapy in which they can participate and assist the patient.
- Visit patients according to the Plan of Treatment; complete a progress note for each visit; and submit progress notes timely.
- Prepare a written discharge summary and a written summary report on a patient’s condition at least every 60 days.
- Current state license as a Physical Therapist.
- Graduate of an American Physical Therapy Association approved physical therapy program.
- Minimum of 1 year of experience in home health.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Excellent time management and organizational skills.
- Valid driver’s license and reliable transportation.
