Kindful Health is seeking an AIDE to collaborate with the RN Case Manager and deliver best-in-class hospice care for a reasonably sized caseload in the Longview area.
You will present at IDT every 2 weeks with updates from the care team and coordinate with the RN Case Manager, Clinical Director and Medical Director to ensure Concierge-level hospice services. A current Certified Nurse Aide license and ongoing education are required.
