• Doctoral degree in Physical Therapy (DPT) from an accredited program.
• Active Washington state PT license required prior to start of assignment.
• BLS/CPR certification required (American Heart Association or equivalent).
• Home health experience required.
• Experience with SOC, evaluations, ROC, recertifications, and subsequent visits preferred.
• Familiarity with home health electronic medical records preferred.
• Previous use of HCHB EMR is a plus.
• Ability to travel within a 50 mile radius for patient visits.
• Business casual dress expected.
