Minimum Requirements- Doctoral degree in Physical Therapy (DPT) from an accredited program.
- Active Washington state PT license required prior to start of assignment.
- BLS/CPR certification required (American Heart Association or equivalent).
- Home health experience required.
- Experience with SOC, evaluations, ROC, recertifications, and subsequent visits preferred.
- Familiarity with home health electronic medical records preferred.
- Previous use of HCHB EMR is a plus.
- Ability to travel within a 50 mile radius for patient visits.
