Nurse Practitioner Services of Texas focuses on providing comprehensive care to nursing home residents enrolled in an Institutional Special Needs Plan. Our clinicians prioritize proactive health management, early identification of condition changes, and enhanced on-site availability to reduce unnecessary hospitalizations and emergency visits. We establish individualized care goals, facilitate communication among patients, families, providers, and staff, and emphasize managing care within the residents' homes wherever possible.

The RN Care Coordinator plays a critical role in overseeing quality care measures and aiding care delivery in collaboration with nurse practitioners.

Role Description

This is a part-time, on-site role based in Longview, TX, for an RN Care Coordinator. The RN Care Coordinator is responsible for facilitating and coordinating care for TIHP members within and outside the facility. Key tasks include tracking quality care measures, ensuring compliance with model of care visit requirements, and supporting nurse practitioners in care delivery. The role emphasizes proactive communication, patient care coordination, and monitoring patient health to promote well-being and avoid unnecessary hospitalizations.

Qualifications
  • Strong clinical skills in patient care, including monitoring and early identification of condition changes
  • Proficiency in care coordination, health management, and developing individualized care goals
  • Effective communication skills to collaborate with patients, families, healthcare providers, and facility staff
  • Ability to track quality care measures and ensure regulatory compliance
  • Previous experience in healthcare, case management, or nursing home settings preferred
  • Active Registered Nurse (RN) license in the state of Texas
  • Strong organizational skills with the ability to prioritize and manage multiple tasks
  • Knowledge of disease progression, advance care planning, and chronic condition management is a plus
