Position SummaryWe are seeking a compassionate and skilled Family Nurse Practitioner to join our clinic team. The FNP will provide primary care services and chronic disease management for patients of all ages in a collaborative, clinic-based setting. This role emphasizes health promotion, disease prevention, and management of complex chronic conditions.
Key Responsibilities- Provide comprehensive primary care and chronic care management to patients across the lifespan, including children, adults, and seniors.
- Perform thorough assessments, including physical exams, patient histories, and diagnostic testing.
- Develop, implement, and monitor individualized care plans for chronic conditions such as diabetes, hypertension, asthma, and cardiovascular disease.
- Prescribe medications and therapies according to state scope of practice and clinic protocols.
- Educate patients and families on disease management, preventive care, and lifestyle modifications.
- Collaborate with physicians, nurses, and other healthcare team members to optimize patient outcomes.
- Document patient care in the electronic medical record accurately and timely.
- Participate in quality improvement initiatives and maintain compliance with regulatory requirements.
- Master’s or Doctorate degree in Nursing (MSN or DNP)
- Current Family Nurse Practitioner (FNP) certification
- Active [State] RN and APRN license
- BLS/ACLS certification
- Experience in primary care, pediatrics, geriatrics, or chronic care management preferred
- Excellent communication, organizational, and clinical decision-making skills
