Position Description
Prime Care Hospice (PCH) is seeking a compassionate and dedicated Registered Nurse Case Manager (RN CM) to join our Longview team. The RN CM is responsible for coordinating and delivering skilled nursing care to hospice patients while serving as the primary clinical liaison between patients, families, physicians, and the interdisciplinary team. This role focuses on promoting comfort, dignity, and quality of life through comprehensive assessments, symptom management, care planning, patient education, and ongoing support throughout the hospice journey.
The ideal candidate is highly organized, clinically skilled, and passionate about providing exceptional end-of-life care. The RN CM must be able to work independently, manage a patient caseload efficiently, communicate effectively with team members and families, and maintain accurate and timely documentation in accordance with Medicare regulations and agency standards.
As a member of the Prime Care Hospice team, the RN Case Manager plays a vital role in ensuring patients receive individualized, compassionate care while supporting families during one of life's most important transitions.

RN Case Manager (RN CM) Job Summary - Longview, PCH
The Registered Nurse Case Manager (RN CM) is responsible for coordinating and managing the comprehensive care of hospice patients throughout the Longview service area for Prime Care Hospice (PCH). The RN CM serves as the primary clinical contact for assigned patients, ensuring quality end-of-life care through ongoing assessments, care planning, symptom management, patient and family education, and interdisciplinary collaboration. This position plays a critical role in supporting patients and families while maintaining compliance with hospice regulations and agency standards.
Key Responsibilities:
- Manage an assigned caseload of hospice patients within the Longview service area.
- Complete admissions, routine nursing visits, recertifications, and comprehensive assessments.
- Develop, implement, and update individualized patient care plans.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, CNAs, and other interdisciplinary team members.
- Provide education and emotional support to patients, families, and caregivers regarding disease progression, symptom management, medications, and hospice services.
- Monitor patient conditions and respond appropriately to changes in status.
- Ensure timely and accurate documentation in the electronic medical record.
- Participate in Interdisciplinary Group (IDG) meetings and contribute to patient care planning.
- Maintain compliance with Medicare Conditions of Participation, agency policies, and regulatory requirements.
- Assist with on-call coverage and after-hours patient needs as assigned.
- Coordinate hospice services to promote patient comfort, dignity, and quality of life.
- Current Registered Nurse (RN) license in the State of Texas.
- Minimum one year of nursing experience; hospice, home health, or case management experience preferred.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently and effectively manage a patient caseload.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
