Job Title: Licensed Vocational Nurse (LVN) – Home Health
Location: Longview, TX 75605 and surrounding 50-mile radius
Duration: 3+ months contract with possible extension
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
On‑Call Requirement: No
Position SummaryWe are seeking a dedicated and experienced Travel Licensed Vocational Nurse (LVN) to join our Home Health team. This field-based role involves providing direct patient care in a home setting, ensuring high‑quality clinical outcomes while maintaining compliance with agency standards.
Key Responsibilities- Provide in‑home patient care in a field set
- Perform comprehensive patient assessment
- Administer and manage wound care, PICC line, Foley catheter and ostomy care
- Conduct and document vital signs monitoring and lab collection (as applicable)
- Accurately document all patient interactions in HCHB (Homecare Homebase)
- Communicate and report any patient condition changes to supervising Registered Nurse (RN) physician
- Maintain compliance with clinical protocols and safety standards
- Active Texas Licensed Vocational Nurse (LVN/LPN) license
- Current CPR certification
- Proven home health field experience
- Proficiency with HCHB (Homecare Homebase) system
- Strong clinical skills in home‑based patient care
- Extensive experience with home health patient care in a field set
- Strong time management and ability to meet productivity targets
- Excellent communication and documentation skills
- Mileage reimbursement:
Yes (between patient visits—from first to last patient) - Dress Code:
Scrubs in Black, Gray or Purple
