You’ll have the opportunity to make a difference in an underserved community and impact the development of primary care services. The Lakefront Family Medicine Clinic includes family practice, same‑day care, and select occupational health screenings.

• Schedule:
  
  1.0 FTE, Full time, 4‑to‑5 days weekly, 34 patient contact hours weekly, with in‑basket nurse practitioner support
• Call: ~1:20, evening phone calls, including one working weekend in the same day clinic once per quarter
• Team: 8 Family Medicine Physicians and ~15 advanced practice practitioners
• 32 exam spaces, shared with same‑day clinicians; located on the campus of the hospital, with on‑site digital X‑ray
• 1:1 medical assistant ratio, seeing all ages, up to ~20 patients per day
• Onsite: social workers, dietitians, clinical pharmacist, diabetic & palliative care specialists, phlebotomists, behavioral health support services
• In‑house controlled substance management, chronic care coordination teams
• Clinician must hold BLS
