Job Description

Peace Health is seeking a Care Management/Medical Social Work for a Per Diem/Relief, 0.00 FTE, Variable position. Salary range: $38.22 – $57.34 (dependent on education, experience, collective bargaining agreement, seniority, and other factors).

Job Summary

Responsible for identifying and interacting with medically and psychosocially complex patients and families who are likely to benefit from care management and meet high‑risk criteria, and for coordinating discharge planning services in collaboration with RN Care Management and other members of the care team.

Details of the Position
  • Screen and identify patients who need care management per high‑risk criteria.
  • Assess, develop, implement, and monitor a comprehensive discharge plan of care through an interdisciplinary team process with the patient and family.
  • Identify key problems, strengths, and resources to be addressed in the discharge plan.
  • Coordinate and facilitate improved compliance with the plan of treatment; provide counseling or support to cope with the situation.
  • Support measures that promote effective use of resources.
  • Plan and arrange for appropriate services, applying knowledge of community, state, and federal health regulations and admission, discharge, and appropriate level of care.
  • Arrange services, education, and provide psychosocial support to prepare the patient and family for management of healthcare needs within the acute care setting and post‑discharge.
  • Coordinate with the interdisciplinary team and community resources regarding transitional care management plan details; consult with physician as indicated.
  • Work with patients identified and referred by RN Care Management and/or other care team members, and by patients/families.
  • Conduct evaluations, including appropriate documentation of Care Management services’ effectiveness; collaborate with team members to adjust the plan if patient health status does not improve.
  • CSupport patients and/or families to facilitate or participate in community care services, coordinating with physicians and treatment teams; assess and address mental health and chemical dependency conditions; perform risk assessments for suicidality and homicidality.
  • Perform other duties as assigned.
What You Bring
  • Bachelor's Degree in Social Work or a related field, with a minimum of four additional years’ work experience in a medical or healthcare setting, social service agency, or community organization focusing on health and/or welfare issues.
  • Master's Degree in Social Work, Counseling, or related field.
  • Minimum of 2 years of preferred employment in a healthcare setting or community agency dealing with health and/or welfare issues.
  • Basic Life Support certification.
  • Certified Case Manager certification (preferred).
  • Within 90 days, obtain Washington State Counselor Agency Affiliate (Washington Requirement: Applied for or received).
  • Upon hire, possess Washington State Social/Counseling Work Credential (Washington Requirement: Other applicable Social Work, Therapist, or Counselor licenses).

  • Excellent verbal and written communication skills, including sensitivity to other cultures and ethnicities (Required).
  • Excellent conceptual thinking, listening, problem resolution, and planning skills (Required).
  • Demonstrated leadership skills (Required).
  • Excellent organizational skills (Required).
  • Proficient computer skills, including MS Office and electronic medical records (Required).
  • Knowledgeable about issues related to chronic illness, developmental disabilities, special needs, mental illness, grief and transition, substance abuse, domestic violence, child abuse, and senior abuse (Required).
  • Good understanding and adherence to core social work values and ethics (Required).
  • Demonstrated knowledge of community health, welfare, and social agencies (Required).
  • Demonstrated knowledge of and ability to apply age‑specific principles of growth and development and life stages to meet each patient’s needs (Required).
  • Demonstrated proficiency in social work practice (Required).
Working Conditions
  • Ability to move around the area with occasional sitting.
  • Bend, twist, reach, push, pull occasionally.
  • Lift/Carry occasionally up to 5 lbs.
Environmental Conditions
  • Exposure to biohazard, body fluids, and airborne particles.
  • Time spent on site in medical/hospital setting or in office environment.
Mental/Visual
  • Vision and hearing required to normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
