Clinical Pharmacy Specialist Ambulatory

Peace Health is seeking a Clinical Pharmacy Specialist – Ambulatory for a part‑time (0.80 FTE), day position in Vancouver, WA or Longview, WA. The salary range is $71.02–$106.54, dependent on education, training, work experience, collective‑bargaining terms, seniority, etc.

Collaboration with Peace Health Medical Group clinicians and staff to provide coordinated, comprehensive, high‑quality medication management care to patients.

• Provide ambulatory‑based services including primary care, specialty care, refill clinics, PA support, pain management, annual wellness visits, anticoagulation services, provider education, comprehensive medication management, and medication safety.
• Deliver evidence‑based medication management using a Collaborative Drug Therapy Agreement.
• Implement and monitor clinical protocols and guidelines, enhancing evidence‑based medicine for services outside the Collaborative Drug Therapy Agreement.
• Serve on and consult to Peace Health Medical Group committees as requested.
• Provide drug‑therapy related education and training to team members and patients.
• Document clinical activities and outcomes.
• Participate in initiatives to enhance pharmaceutical care and medication safety.
• Detect, monitor, and report adverse drug events and medication errors.
• Oversee orientation, training, and mentorship of pharmacy students, residents, and technicians.

• Bachelor of Science in Pharmacy from an APCE‑accredited school (required)
• Doctor of Pharmacy (preferred) or BSc Pharmacy or Board of Pharmacy Specialties Certification in ambulatory care (BCACP, BCGP, etc.) or completion of a PGY2 residency (preferred)

• Minimum 1 year of pharmacy experience in ambulatory care (required)
• Comprehensive knowledge of evidence‑based clinical pharmacy practice in ambulatory care specialties (required)
• Minimum 3 years of advanced pharmacotherapy specialist practice in ambulatory clinical pharmacy focusing on direct patient care, medication management, consulting, and teaching (preferred)

• Licensed pharmacist in Washington State (required)
• Board of Pharmacy Specialties certification in ambulatory care within 2 years of hire (required)
• Anticoagulation certification (ASHP Anticoagulation Certificate, University of South Indiana Anticoagulation Therapy Management Program Certificate, or CACP) within 6 months of hire if not already Board certified (required)
• Basic Life Support upon hire (required)

• Meet payor credentialing requirements for pharmacist billing of clinic services (required)
• Practice under a Collaborative Drug Therapy Agreement (required)
• Implement, document, and monitor anticoagulation management (required)
• Proficiency with computers and pharmacy systems (required)
• Excellent verbal and written communication and presentation skills (required)
• Collaborate effectively within an interdisciplinary team (required)
• Precept and obtain necessary certifications for pharmacy students and residents (required)
• Support, coach, mentor, and lead staff (required)
• Strong leadership and organizational abilities (required)

• Lifting: occasionally up to 30 lbs., push/pull up to 14 lbs. force
• Physical movements: moving around area, occasional sitting, squatting, kneeling, bending, fine‑motor activities, frequent reaching
• Environmental: exposure to biohazard, body fluids, airborne particles, and ability to work in a noisy environment
• Mental/Visual: vision and hearing within normal limits (glasses, contacts, hearing aids permitted); ability to communicate and exchange accurate information
