Physical Therapist - Home Health

Role:

The Physical Therapist in Home Health evaluates patients' functional status and physical therapy needs, assists in developing treatment plans, and provides interventions to improve or maintain patient well-being.

• Assess patient condition and contribute to therapy plan development in collaboration with physicians.
• Deliver therapy services within legal scope and according to plan of care, coordinating with healthcare team.
• Monitor and report patient responses and condition changes.
• Treat patients to alleviate pain, restore function, and maximize mobility.
• Educate patients, families, and healthcare team members about care plans, progress, and limitations.
• Complete evaluations, develop plans of care, and document progress in compliance with guidelines.
• Report outcomes, goals, and care frequency estimates.
• Operate effectively in diverse home environments, respecting all patients regardless of background.

• Current Physical Therapy licensure in the relevant state.
• Valid driver's license, vehicle insurance, or reliable transportation.
• Preferred CPR certification or ability to obtain it within 90 days.
• Strong assessment, communication, and patient education skills.
