We are seeking a Nurse Practitioner to join Cowlitz Family Health Center, a Federally Qualified Health Center (FQHC) that provides primary care and preventive services to underserved populations.

• Deliver primary care medical services, including diagnosing medical conditions, prescribing medications, and monitoring medication side effects.
• Provide preventive health treatments.
• Collaborate with a multidisciplinary care team consisting of medical assistants, receptionists, care coordination staff, behavioral health specialists, and outreach personnel.
• Maintain comprehensive patient care within a team-based environment.
• Engage patients in improving and maintaining their health.

• Active licensure to practice in the State of Washington, without restriction, probation, or limiting condition.
• Board certified or board eligible in Family Practice, Internal Medicine, or Maternity/Pediatrics.
• Valid, unrestricted license to prescribe drugs, medications, pharmaceuticals, and controlled substances.
• At least two years of experience providing primary care in an outpatient setting.
• Knowledge of evidence-based primary clinical care standards.
• Experience working with underserved and transient populations.
• Bilingual in English/Spanish.
• Proficient computer skills and experience with electronic health records.
• Strong communication skills and ability to collaborate effectively in a team setting.

Annual Salary: $131,100 - $139,050
Full-Time Equivalent (FTE): 1.010 hour shifts
