Eden Health - - Responsibilities:
Provides care ordered by the physician, including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding and transferring the patient.;
Takes accurate vital signs.;
Documents clearly in the medical record as relates to reimbursement guidelines and Agency policy and procedure.;
Identifies and communicates findings to the Case Manager and/or Clinical Manager.;
Works as part of the interdisciplinary team and supports hospice regulations and philosophy.
